Participation Agreement
Fees: A $25 non-refundable deposit is due at the time of registration by November 10th to ensure a costume is available. The remaining balance of $50 will be due on the first day of camp. Remaining balance can be paid with cash or checks made out to PMDI.
Injuries: Parents or legal guardians of minor students and adult students waive the right to any legal action for any injury sustained on school property resulting from normal dance activity or any other activity conducted by the student(s) before, during or after class time.
Photo Release: The school is hereby granted permission to take photographs and video of the students or use yearly class and individual photos in brochures, websites, posters, advertisements and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs and video in its name.
Costume Adjustment: The studio reserves the right to make any adjustments to the costume selection if necessary.
